What is a UDID card?​

  • The UDID Card (Unique Disability Identity Card) simplifies access to government benefits for people with disabilities.
  • This government initiative creates a national database and issues a unique ID card to each person with a disability.
  • It improves transparency, efficiency, and ensures everyone receives benefits fairly.
  • The UDID system also allows for better tracking of how these programs are helping people at all levels.
Eligibility of UDID card ?

21 types of disabilities is recognised by UDID and Only those with the specific types of listed disabilities can apply for a UDID card​

  • Movement: Locomotor disability ( leprosy-cured persons, cerebral palsy, dwarfism, muscular dystrophy)
  • Acid Attack Victims
  • Vision: Blindness, low vision
  • Hearing: Deafness, hard of hearing
  • Speech and Language
  • Intellectual Functioning: Intellectual disability (including specific learning disabilities, autism spectrum disorder)
  • Mental Health: Mental illness
  • Chronic Neurological Conditions: Multiple sclerosis, Parkinson's disease
  • Blood Disorders: Hemophilia, Thalassemia, Sickle cell disease
  • Multiple Disabilities: Combinations of the above, including deafblindness
  • Future Categories: As notified by the government

Benefits of a UDID card?​

  • One card, many benefits: The UDID card replaces the need for multiple disability documents, making access to government programs easier.
  • Unlocks government support: This government-recognized ID allows people with disabilities to access various benefits like financial aid, healthcare, education, and job opportunities.
  • Faster and fairer help: A unique ID number simplifies identification, ensuring PwDs receive the right assistance efficiently.
  • Building accessibility: Information on the card helps create more accessible public spaces and transportation for people with disabilities.
Documents required for applying for the UDID card:-
  • Photo: A recent colour photograph.
  • ID Proof: Aadhar Card (or any government-issued ID like PAN card or Passport).
  • Address Proof: Passport, Aadhar Card, PAN Card, or utility bill
  • Signature/Thumbprint (Optional): A scan of your signature or thumbprint.
  • Disability Certificate (Optional): If you don't have one, you can apply for a new one during the UDID card application process.
Validity of a UDID Card:-
The UDID card has two types of validity-
  • Permanent UDID card: Valid for life, issued for disabilities unlikely to improve.
  • Temporary UDID card: Valid for a specific period, issued for disabilities that may change.
Different types of UDID Cards:-
Three types of UDID Cards are issued based on the severity of the disability:-
  1. White Card: Issued for disabilities with a severity of less than 40%.
  2. Yellow Card: Issued for disabilities with a severity between 40% and 80%.
  3. Blue Card: Issued for disabilities with a severity of greater than 80%.

In this portal you can do the following things:-
  • Apply for Disability Certificate & UDID Card
  • Apply for Disability Certificate & UDID Card Renewal
  • Apply for Lost UDID Card
  • Track Your Application Status
  • Download your e-Disability Card & e-UDID Card
  • Update Personal Profile

How to Apply/ Renew a UDID Card ?​

  1. Visit swavlambancard portal for a fresh UDID card or for a renewal of UDID card
  2. After the application is submitted in the portal, the application is sent to the notified Chief Medical Officer (CMO) or District Medical Officer (DMO) of the applicant’s resident district
  3. After the verification of application is completed , CMO/DMO assigns specialist doctors for assessment of the applicant
  4. The specialist doctor assesses the applicant (Person with Disability ) and then submits the assessment report to the medical Board
  5. The Medical Board takes decision on the percentage and type of disability, validity of disability certificate and other such issues of the applicant. The medical board informs its decision to the concerned CMO/DMO.
  6. A UDID card is generated based on the assessment details completed by the CMO/DMO.
  7. The UDID card is then dispatched to the applicant through Speed Post.
How to Track the Application Status of a UDID card?
  1. Visit swavlambancard website (***********.swavlambancard.gov.in/)
  2. Click the “Track your Application Status “ option
  3. Enter the unique application reference number provided to you during your UDID card application submission then enter the captcha and the security question.
  4. After that , click on the 'Check Status' to initiate the tracking process.
  5. The portal will display the status of your UDID card application.
How to Download your UDID Card ?
  1. Visit swavlambancard website (***********.swavlambancard.gov.in/)
  2. Click the “get your e-disability card and e-UDID” option
  3. enter your enrolment number, birthdate, and captcha code on the login page and click on the Login Button
  4. You will see the e-disability card and e-UDID card
  5. select the download option and the cards would be downloaded
You can also download your UDID card from the Digi Locker

How to Apply for your Lost UDID Card ?
  1. Visit swavlambancard website (***********.swavlambancard.gov.in/)
  2. Click the “Apply for your Lost UDID card” option
  3. enter your enrolment number, UDID number ,birthdate, and captcha code and click on the submit Button.
  4. You have to now upload the FIR copy and again fill the captcha code and click on the submit button.
  5. You will get a new UDID card
Optional - you can also download the e-UDID and then print it out , it will work the same.
